What you'll need

  • Proof of Insurance :

    • Minimum $100,000 cargo insurance

    • Minimum $1,000,000 auto liability

    • Insurer with B+ or better A.M Best Rating

  • Details about your modes, commodities, tractor/equipment counts and certification

  • Operating authority with the below requirements

    • Valid interstate or intrastate authority (intrastate-only carrier must allso have an active U.S. DOT #)

    • Satisfactory or NOT rated by the DOT

    • If NOT rated by the DOT, you will be subject to additional qualification procedures

    • Must be in business for 12 months

  • Completed electronic Master Transportation Contract

  • Completed W-9 form for tax purposes
